This web application is based on surveying some action which is required in organization. This application has ability to create more than one dynamic form using required elements. Using this we can assign form to list of application user whose submit required action on that form. This application has versioning functionality using that we can manage our program version. Using email notification we can send email notification to assigned form user they can complete his/her assigned form in particular time duration. We can assign form according to user and group with sequence and without sequence. We generate report for the submitted form or action taken by user or group
